The presentation, “Preventing Relapse in MH&A Recovery: How Axon Health is Reinventing Aftercare via Remote Monitoring” addresses a persistent yet under-served challenge in mental health: the lack of effective, scalable aftercare to prevent relapse post-treatment. Globally, up to 85% of individuals with substance use disorders relapse within the first year following treatment based on a NIH study. Despite significant investment in primary treatment programs, aftercare services remain fragmented, non-personalized, and reactive. Many clinics struggle to stay engaged with clients post-discharge, leading to poorer outcomes, increased costs, and preventable readmissions. For populations in rural or underserved areas, access to structured aftercare is even more limited. Axon Health introduces a Remote Therapy Monitoring (RTM) platform which is designed specifically for outpatient and residential mental health and addiction recovery settings with the aim to bridge this aftercare gap. While digital mental health tools have proliferated, few are tailored for the critical post-treatment phase. This presentation introduces a novel framework where real-time patient engagement, daily self-monitoring, and outcome-driven insights are integrated into a seamless aftercare experience. Residential rehabs using Axon Health gain: 1. An automated, structured, and personalized approach to continuing care 2. Data-driven insights that enable early intervention before relapse occurs 3. Enhanced therapist-client communication via secure digital channels 4. A scalable solution that improves outcomes while reducing clinician burnout Presentation Structure 1.
